Join a leading acute-care hospital as a Travel RN Case Manager in Paramount, CA. This contract position offers an exciting opportunity to coordinate management of care and ensure optimum utilization of resources while delivering high-quality patient care.
As a Travel RN Case Manager, you will provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. Your role will enhance the quality of patient management and satisfaction, promote continuity of care, and ensure cost-effectiveness through the integration of case management, utilization review, and discharge planning functions. You will serve as a patient advocate, addressing the holistic needs of patients while ensuring their preferences are respected.
Facility Requirements:
- Must have BLS certification (must be attached to submission packet and through the American Heart Association)
- Must have a minimum of 2 years of recent RN Case Management experience, with experience in a LTAC or STACH setting, MCG, and discharge planning across acute care settings
- Strong interdisciplinary team (IDT) collaboration skills
- Must have a valid California RN license (must be attached to resume)
- Shift will be Monday - Friday, 8:00 AM - 4:30 PM
