As a Case Manager RN, you will be responsible for coordinating patient care, ensuring quality outcomes, and facilitating communication among healthcare providers. You will need to have at least 2 years of acute care experience, and first-timers are welcome with clinical approval. This position requires you to work every other weekend and may require floating between like units based on need.
Key Responsibilities:
- Coordinate patient care and manage case loads effectively.
- Communicate with healthcare team members to ensure quality patient outcomes.
- Document patient progress and care plans using Epic charting.
Requirements:
- 2 years of acute care experience required.
- Weekend work required (every other weekend).
- First-timers accepted with clinical approval.
- Must be able to float between like units as needed.
