As a Home Care RN specializing in Hospice, you will join a community-focused, not-for-profit organization dedicated to enhancing the well-being of individuals in Central Vermont. This position is responsible for providing individualized goal-oriented nursing care using the nursing process in accordance with a physician's order. In addition, this position serves as Case Manager in cases in which this position is the lead discipline.
