  • Pay Rate: $17.00/hour
  • Weekly Gross Pay Range: $1,800 - $1,900
  • Weekly Housing Stipend: $770

Key Responsibilities:

  • Provide high-quality nursing care to residents, including vital sign monitoring, medication administration, wound care, and assistance with activities of daily living (ADLs).
  • Accurately document patient health histories, monitor changes, and update electronic medical records in accordance with facility policies and state regulations.
  • Collaborate with RNs and the healthcare team to develop and implement individualized care plans.
  • Report significant changes in resident health or behavior.
  • Supervise and mentor CNAs, ensuring adherence to care plans and standards of safe, compassionate care.
  • Act as a liaison among residents, families, and medical teams, effectively communicating care updates and treatment adjustments.
  • Maintain strict compliance with all facility policies, infection control protocols, and federal/state regulations.

Qualifications:

  • Current, active LPN license valid in New York State.
  • Graduate of an approved nursing program.
  • Minimum of 1 year of experience in a skilled nursing or long-term care setting preferred.
  • BLS/CPR certification current and valid.
  • Strong communication and interpersonal skills, with the ability to multitask and remain professional under pressure.
  • Physical ability to stand, walk, bend, and lift/move up to 50 lbs as needed.
