Position Summary
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our nursing team for the evening shift. In this role, you will be the primary caregiver for our residents, providing essential medical care, monitoring health status, and ensuring that our residents’ daily needs are met with dignity and respect. You will work closely under the supervision of a Registered Nurse (RN) and provide leadership to our Certified Nursing Assistants (CNAs).
Key Responsibilities
- Direct Patient Care: Provide high-quality nursing care to residents, including monitoring vital signs, administering medications, performing wound care, and assisting with ADLs (Activities of Daily Living) when necessary.
- Documentation: Accurately record patient health histories, monitor changes in physical or mental status, and update electronic medical records in compliance with facility policies and state regulations.
- Clinical Collaboration: Partner with the RN and interdisciplinary team to implement and carry out resident care plans. Report any significant changes in resident health or behavioral status to the charge nurse or physician.
- Leadership: Supervise and mentor CNAs, ensuring they are following care plans and providing safe, compassionate care.
- Communication: Act as a liaison between residents, their families, and the medical team, ensuring that all parties are informed of care progress and any adjustments to treatment.
- Compliance: Maintain strict adherence to all facility policies, infection control protocols, and State/Federal regulations.
Qualifications
- License: Current, active LPN license valid in the state of practice.
- Education: Graduate of an approved school of nursing.
- Experience: Prior experience in a skilled nursing or long-term care setting is highly preferred.
- Certifications: Current BLS/CPR certification.
- Skills: Strong communication, interpersonal, and multitasking skills. Ability to remain calm and professional under pressure.
- Physical Requirements: Must be able to stand, walk, bend, and lift/move up to 50 lbs as needed for patient care.
