Join a leading healthcare organization as an Outpatient Clinic Registered Nurse in New Haven, CT. This role is perfect for RNs with a passion for patient care in an outpatient oncology setting. You will play a vital role in managing patient communications, providing education, and coordinating care within a multidisciplinary team.

Role Includes:

  • Managing and responding to patient messages through the electronic in-basket.
  • Providing patient education related to oncology diagnoses, treatment plans, medications, and follow-up care.
  • Performing phone triage and determining appropriate follow-up or escalation in the outpatient oncology clinic setting.
  • Care coordination: tests, referrals, appointments, and other aspects of the patients' plan of care.
  • Supporting clinic workflows for a designated outpatient oncology team (assisting with documentation, care coordination, patient education, and follow-up needs).
  • Collaborating with the multidisciplinary oncology team to address patient needs and maintain continuity of care.
