GLC Group is seeking a Licensed Practical Nurse (LPN) for PRN/Per Diem shifts at a skilled nursing facility in Worthington, PA.

Pay & Schedule:

  • Hourly Rate: $40
  • Weekly Pay – Every Friday
  • Flexible PRN shifts

Key Responsibilities:

  • Administer medications and treatments as prescribed.
  • Monitor and document residents' health status and changes in condition.
  • Provide direct nursing care and support with daily living activities.
  • Assess residents and report concerns to the RN, physician, or care team.
  • Develop, implement, and update resident care plans.
  • Supervise and delegate tasks to nursing assistants and support staff.
  • Maintain accurate and timely medical records.
  • Respond to emergencies and provide appropriate interventions.
  • Educate residents and families on care plans and health needs.
  • Ensure compliance with facility policies, safety standards, and regulatory requirements.

Requirements:

  • Active LPN license in Pennsylvania
  • Minimum 1 year of recent LTC/SNF experience
  • BLS (AHA) required; ACLS preferred
  • COVID-19 vaccination or approved exemption, if applicable
