Department Information:
o Average census and case load/day - 99% capacity on most days; 14-16 patients - dependent on unit assigned
o Minimum Years of Experience required - At least 3-5 years in Acute Care Case Management - please decline if this is not their most recent experience
o Will you accept a first-time traveler - No, must have traveled as an RN-Case Manager
Patient Population:
o Dependent on unit - Adult Medical/Surgical, Cardiac, Ortho and possibly Peds
Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
o RN: BSN required; BLS; Case Management certification (preferred)
Skills required:
o RN:
EPIC experience is required
Skilled in placement for SNF/IPR/LTAC/Hospice/Home Health, DME, Behavioral Health needs/DSS (peds), Difficult complex placement, will work routinely with homeless population
Care management skills: assessment, care coordination and discharge planning
Support within the Department:
o Director
o Manager(s)
o CMO
o UM team that is very supportive - PA is part of this team
o Peer CMs to support
o Social Worker(s)
Technology/Equipment:
o EMR: EPIC - required
o Rover Chat in EPIC and will carry a phone
o Careport - referral management (similar to Allscripts) - only preferred experience
Floating:
o Possible to any unit
Orientation (facility and unit) :
o Will vary based on skillset/comfort level - 2-5 days departmental
Scheduling:
o Weekend rotation: no weekends
o No Call
o Holiday Expectations: Travelers will work all holidays unless approved off in contract
o RTO: Manager approval
o Shift times: Will vary based on requisition
RN:
8h shifts - will work (5) 8s with an 0800 start
