GLC is hiring: RN Case Manager needed at a leading acute-care hospital in Bronx, NY 10457. This role is ideal for a dedicated professional with a passion for patient care and case management.
Pay Details: $2,610/week
Locals Welcome: $58/hr $86/hr OT
Shifts: Mon-Fri 8:00 AM - 4:30 PM, 7.5 hours shifts with 1-hour lunch
Details: 1 year of inpatient Case Management experience is needed. Trauma Level 2 certifications required: BLS, PRI (Patient Review Instrument) Certification & SCREEN Training Certification. These certifications can be taken online through the School of Albany, typically taking a couple of hours for each certification at an approximate cost of $300/certification. Type of Patients: Inpatient - Common types of patients & job duties include discharge to home, community, or SNF. Responsibilities include obtaining housing, shelter packets for homelessness, hemodialysis packets, KCI referrals for wound vacs, and more.
Experience or 'Must have' skills: Must have 1 year of inpatient case management experience. Prior critical care experience is preferred. The role requires knowledge of discharge planning and making referrals. You will typically work on a Med-Surg or Tele unit and split the floor with a Social Worker. Candidates must be able to function independently on the unit and work collaboratively with an interdisciplinary team and patients for discharge planning. Rounding on the unit with physicians/team is expected. A positive attitude, critical thinking skills, and good time management are essential. Note that Utilization Review (UR) is handled by a separate department.
Caseload: Floors typically have 33-40 patients; most floors are split with 1 Case Manager & 1 Social Worker; caseload: 17-25 patients. Pilot floors will have 1 Case Manager & 2 Social Workers; caseload: 12-17 patients. Candidates need to be able to handle at least 20 patients. Typically placed on Med-Surg or Tele Floors following Internal Medicine and Family Medicine.
Department Specific Orientation: 2 days of hospital orientation will include EMR training; 1-2 weeks with a preceptor depending on how quickly candidates can be checked off. EMR used: Allscripts. Department: Case Management.
Required Certifications: BLS. Other certifications/specialty certification requirements include PRI & SCREEN certifications (details as mentioned above).
