Join a leading healthcare provider as a dedicated LVN/LPN in Long Term Care (LTC/SNF) in Kane, PA. The LPN Wing Supervisor will work under the direction of the RN Supervisor, carrying out the medical plan of care and overseeing the Nursing Assistants' care of residents and high school students to ensure that resident care is delivered in accordance with best nursing practices and applicable State and Federal regulations. The ideal candidate will have a passion for working with and interacting with residents daily.
General Duties
- Administers and records medication, treatments, and monitoring (e.g., glucose checks) as ordered, reporting and documenting resident responses.
- Accurately transcribes physician's orders and completes monthly summaries as scheduled.
- Reviews and updates care plans with Nursing Assistants on monthly summary day to reflect residents' current conditions.
- Notifies the RN Supervisor immediately of any significant changes in residents' conditions.
- Communicates resident transfers to immediate family and department heads via email.
- Assists physicians with rounds, examinations, and treatments as needed.
- Collects laboratory specimens and arranges for delivery to the hospital with accurate requisitions.
- Encourages resident participation in scheduled activities and assists with post-mortem procedures when necessary.
- Maintains a clean, safe, and healthy environment and reports potential safety hazards.
- Supervises and directs resident care by nursing assistants to ensure the prescribed plan of care is carried out.

- Uniforms: Any colored scrubs
- Shoes: Clean and in good repair (no open-toe sandals)
- Hair: Clean and neat
- Jewelry: No large or dangling earrings or necklaces
- Name tags: Worn daily
Physical Demands
- Good physical health and mental alertness to perform job functions in a timely manner.
- Ability to pass employment physicals as required.
